How to use this QR generator
Share a survey or registration form on a printed sign without asking visitors to type its address. This Google Forms QR generator encodes the responder link you supply. It does not create a form, connect to your Google account or collect responses; those tasks stay with your form provider.
- Publish your form and copy its responder link. Use the address for filling out the form, rather than its editing link.
- Paste the link into Website address. Open it in a private window to check who can respond and whether a Google sign-in is required.
- Download your QR and scan it on a phone. Complete a test response before putting the code on a registration sheet or feedback sign.
Responder permissions determine access
A QR code makes a link convenient to open; it does not change the form's permissions. Google Forms must be published for responders to access it. A form limited to one response requires Google sign-in, and an organization may restrict access further. Check the settings against your intended audience, especially for a public event. Avoid encoding an editor or collaborator URL.
Keep the form link stable
Changes to questions at the same responder URL can be seen by future visitors without changing the QR. A new URL requires a new static code. Make QR does not shorten or track form links. Give your sign a clear action such as 'Scan to register' and include a readable alternative for anyone who cannot scan. Other hosted survey and form URLs work in the URL generator too.

Frequently asked questions
Can I generate a Google Form here?
No. Create and publish the form with Google Forms first, then paste its responder URL here. This tool generates the QR artwork only.
Will a scan submit a response?
No. The visitor opens the form and chooses what to submit. The form provider handles responses and its own privacy settings.
Is this an official Google tool?
No. Make QR is independent and does not access your Google account. Google controls the linked form and responder permissions.
